Looking for an all-in-one plan that includes Long-Term Care services?

CompleteCare (HMO D-SNP)

This all-in-one Medicaid Advantage Plus plan includes both Long-Term Care services and medical health coverage. So members are covered for in-home care PLUS doctor visits, prescription drug coverage, and a $170 a month over-the counter (OTC) allowance to spend on health-related items, healthy foods, fitness equipment and trackers, or even home Internet service.

Senior Health Partners MLTC Medicaid Plan

This plan adds access to in-home Long-Term Care services to existing health coverage. You would use your Medicare health coverage to access medical, hospital, and prescription drug coverage.

*Healthfirst will cover non-emergency Medicaid-covered transportation provided that it is included as a Managed Long-Term Care benefit by the New York State Department of Health.

**If you qualify for Medicaid, your monthly premium will be $0.

If you receive Medicaid and want to know whether you are eligible for a Managed Long-Term Care Plan, you can call the New York Independent Assessor (NYIA) formerly Conflict-Free Evaluation Enrollment Center (CFEEC) at 1-855-222-8350, TTY at 1-888-329-1541, Monday to Friday, 8:30am-8pm, and Saturday, 10am-6pm, to schedule your initial assessment.

Coverage is provided by Healthfirst Health Plan, Inc. Plans contain exclusions and limitations.

Coverage for Senior Health Partners, Managed Long-Term Care Plan, is provided by Healthfirst PHSP, Inc.

Plans above reflect amounts after Extra Help or Medicaid secondary coverage has been applied.

Healthfirst Health Plan, Inc. offers HMO plans that contract with the Federal Government and has a contract with New York State Medicaid for its Dual-Eligible Special Needs Plans. Enrollment in Healthfirst Medicare Plan depends on contract renewal.

You must continue to pay your Medicare Part B premium, if applicable.
