Essential Plans

For qualified individuals and eligible immigrants age 19-64, depending on your income and immigration status. For a $0 monthly premium, these plans offer essential health benefits, plus dental and vision coverage.

Medicaid Managed Care

For low-income families, and for individuals under 65, who qualify for Medicaid in New York State. This high-quality plan offers benefits that include maternity coverage, health and wellness programs, and more. All for a $0 monthly plan premium.

Child Health Plus

For uninsured or underinsured families who do not qualify for Medicaid. This Children's Health Insurance Program (CHIP) plan offers no-cost or low-cost coverage for qualified children under age 19, regardless of family income or immigration status.

Marketplace Plans

For families and for individuals under the age of 65. These plans offer health benefits with various options based on your needs and budget. Financial assistance is available. (These plans are also called Qualified Health Plans or Healthfirst Leaf Plans.)

How do I receive financial help?

Your income, family size, and the plan you select will determine if you’re eligible to get help paying for your health insurance. To find out if you qualify, you can contact us by phone or fill out a form to have us contact you. A representative will help you understand your health plan and subsidy options.

Yes. At Healthfirst, our representatives speak Spanish, Chinese, Russian, and many other languages. Visit our Virtual Community Office to select a representative who speaks your language.
